Diabetic Neuropathy: Causes, Symptoms, Types, and How to Slow Its Progression
Diabetic neuropathy — nerve damage caused by diabetes — is one of the most common and debilitating complications of diabetes mellitus. It affects approximately 50% of people living with diabetes, significantly reducing quality of life while contributing to serious morbidity and mortality. Between 20 and 30 percent of those affected also develop neuropathic pain. [1]
If you or a loved one has diabetes, understanding diabetic neuropathy — what causes it, what symptoms to watch for, and how to manage it — is essential for protecting your long-term health.
What Causes Diabetic Neuropathy?
Diabetic neuropathy develops from damage to the nerves due to high blood glucose (blood sugar) levels. High blood sugar harms nerves through several distinct mechanisms: [2]
• Oxidative stress: An imbalance between free radicals and antioxidants in the body leads to cell damage, injuring nerve fibers.
• Chronic inflammation: High blood sugar promotes systemic inflammation that accelerates nerve damage.
• Advanced Glycation End Products (AGEs): These harmful compounds accumulate in tissues and can directly damage nerves and the blood vessels that supply them.
Diabetic Neuropathy Symptoms: What to Look For
Diabetic neuropathy symptoms vary widely. Some patients experience painful, disabling symptoms, while others have only mild signs. Depending on which nerves are affected, diabetic neuropathy may cause pain and numbness in the legs, feet, and hands, as well as problems with the digestive system, urinary tract, blood vessels, and heart. Types of Diabetic Neuropathy
1. Peripheral Neuropathy (Distal Symmetric Peripheral Neuropathy)
Peripheral neuropathy is the most common form. It affects the peripheral nervous system, causing pain and numbness in the extremities. Symptoms typically begin in the toes and feet, gradually progress up the legs, and later affect the hands and fingers.
Because peripheral neuropathy can cause numbness, a person with diabetes may not feel a foot injury, cut, or blister — which can lead to a serious ulcer. Diabetic foot ulcers are a considerable problem for people with diabetes. Left untreated due to diminished sensation, a diabetic foot ulcer can worsen over weeks or months without the person knowing, because they do not feel the pain. Diabetic foot ulcers account for more hospital admissions than any other complication of diabetes. [4]
Additional peripheral diabetic neuropathy symptoms include: [5]
• A tingling or burning sensation
• Sharp, jabbing pain that may worsen at night
• Serious discomfort from touch
• Muscle weakness
2. Proximal Neuropathy
Proximal neuropathy causes pain and numbness in the upper legs — specifically the buttocks, thighs, and hips. This type of nerve damage typically affects one side of the body and may rarely spread to the other side. It is more common in men than in women and more common in people over the age of 50. Most people with this condition have Type 2 diabetes. [6]
3. Autonomic Neuropathy
Autonomic neuropathy occurs when nerve damage interferes with signal processing between the autonomic nervous system — which controls automatic body functions such as blood pressure, bladder function, and digestion — and the brain. [7]
Autonomic diabetic neuropathy symptoms include:
• Rapid heart rate at rest
• Dizziness when standing up (orthostatic hypotension)
• Drops in blood pressure
• Sweat gland dysfunction, leading to excessive sweating or an inability to sweat, and difficulty regulating body temperature
• Changes in bowel habits, including constipation or diarrhea
• Frequent indigestion and poor appetite
• Loss of bladder control or difficulty emptying the bladder
• Trouble swallowing
4. Focal Neuropathy
Focal neuropathy damages a single, specific nerve, which may be located in the face, torso, arm, or leg. Unlike the other types, focal neuropathy tends to come on suddenly and affect a localized area.
Can Diabetic Neuropathy Be Reversed?
This is one of the most frequently asked questions about the condition — and the honest answer is no. You cannot reverse neuropathy caused by diabetes, since the body cannot naturally repair damaged nerve tissue. Despite extensive research, effective treatments to halt or reverse the progression of diabetic neuropathy remain difficult to achieve. Alleviating symptoms and controlling pain remain the primary management strategies. [8]
However, research is advancing. Among the latest studies are those identifying proteins as possible drug targets in diabetic neuropathy, focusing on their role in polyol metabolism, oxidative stress, and cytokine regulation. [9] Additionally, early diagnostic markers and advanced imaging techniques — including corneal confocal microscopy and skin biopsy — may enable timelier interventions. Novel pharmacological agents have also shown promising neuroprotective and pain-management effects. [10]
How to Slow the Progression of Diabetic Neuropathy
While you cannot reverse diabetic neuropathy, you can take meaningful steps to slow its progression. The foundation of diabetic neuropathy self-care is keeping blood glucose levels within your target range — because staying close to your goal numbers is the single most effective way to prevent nerve damage from worsening. [11]
Additional self-care strategies include:
• Managing blood pressure to protect both nerves and blood vessels
• Eating a healthy diet rich in vegetables, fruits, and whole grains
• Maintaining a healthy weight to reduce metabolic strain
• Staying physically active — the American Diabetes Association recommends 150 minutes of moderate-intensity exercise per week to help manage diabetes [12]
• Quitting smoking, which worsens circulation and nerve health
• Managing stress, which can raise blood sugar levels
• Limiting alcohol consumption
• Treating injuries and infections promptly before they worsen
• Attending physical therapy for strength, balance, and mobility
• Wearing orthopedic braces to compensate for weakness or loss of balance
• Correcting vitamin deficiencies, particularly B vitamins that support nerve health

Diabetic neuropathy is a serious but manageable condition. You cannot reverse it, but with consistent blood sugar control and proactive self-care, you can slow its progression and protect your quality of life.
